Regulatory Incident Reporting (Article 62)

The EU Artificial Intelligence Act (Article 62) mandates that providers and deployers of high-risk AI systems report serious incidents or functional failures to the relevant national competent authorities. These reports must be filed within strict deadlines (e.g., within 15 days of the operator becoming aware of a serious incident).

Qarion provides a dedicated Regulatory Incident Reporting module integrated directly into the platform's Issue Tracking workflows.

Submitting an Incident Report

When a serious malfunction or harm event occurs related to an AI system in the Qarion registry, compliance officers and data stewards can log a Regulatory Incident.

  1. Navigate to the specific AI system's detail page in the Data Catalog.
  2. Select Issues and click New Regulatory Incident.
  3. Fill out the structured incident payload which captures:
    • Date and time of the incident
    • Detailed description of the malfunction or harm caused
    • Affected individuals or systems
    • Implicated AI models and dependent data pipelines

Deadline Tracking

Given the strict regulatory deadlines for notifying authorities, Qarion automatically calculates and displays the submission deadline.

  • A Countdown Timer appears on the incident dashboard tracking the remaining days (out of the standard 15-day window).
  • Warning alerts are escalated to assigned compliance officers as the deadline approaches.

Follow-up Action Logs

Regulatory reporting is rarely a single event. Authorities frequently require follow-up documentation regarding immediate mitigation steps and long-term root-cause remediation.

  • The Follow-up Log allows responders to securely attach chronological updates to the main incident ticket.
  • Options include logging internal investigations, mitigation actions taken, and communications with the national competent authority.
  • These logs are immutable and form part of the official audit trail.

Generating PDF Reports for Authorities

Once an initial report or a follow-up investigation is complete, Qarion can compile the structured data into a formal submission document.

  • Click Export Report on the incident detail page.
  • Qarion generates a standardized, formatted PDF detailing the AI system's metadata (extracted via data lineage), the incident parameters, and the timeline of mitigation actions.
  • This PDF is designed to be easily attached to official authority portals or sent via secure channels.
