Survey Templates
Survey templates define structured forms for collecting additional information during change requests. Space administrators can create and manage templates, then assign them to specific products.
Overview
Survey templates enable standardised data collection when users submit change requests for data products. Instead of relying on free-text descriptions, teams can define specific fields — dropdowns, text inputs, and more — ensuring consistent information is captured.
Managing Templates
Creating a Template
- Navigate to Space Settings → Survey Templates
- Click New Template
- Add a name and optional description
- Define the form fields:

- Mark fields as required or optional
- Save the template
Editing a Template
Templates can be updated at any time. Changes affect future change requests — existing submissions retain their original form structure.
Deactivating a Template
Deleting a template performs a soft deactivation. The template is hidden from selection but existing references are preserved. To view deactivated templates, toggle the Include inactive filter.
Assigning to Products
Once a template is created, it can be assigned to individual products:
- Navigate to the product's detail page
- Open Settings → Change Request Configuration
- Select a survey template from the dropdown
- Save — all future change requests for this product will include the survey form
Removing a Template Assignment
Clear the template selection in the product settings. Change requests will revert to the default free-text format.
How It Works in Practice
When a user creates a change request for a product with an assigned survey template:
- The change request form displays the survey fields
- Required fields must be completed before submission
- Responses are stored with the change request
- Reviewers can see the structured survey data alongside the request
